Something similar has happened a few days ago, which by the way was one of the most embarrassing moment in the entire Counter Strike history.

Jaryd “summit1g” Lazar, who was recruited on short notice by team Splyce, was playing his debut match with the team. In one of the rounds, following a clean sweep of the opponents, in an attempt to celebrate with his teammates, he walked on his own Molotov with only 40 hp remaining. Splyce ended up not only losing the round, their opponent CLG grabbed the opportunity and made a comeback and eventually won with a margin of 19-15 in the overtime. They latter went on to take the second map, which resulted in putting Splyce at the bottom of the group and eliminated from the tournament.
